WORLD REPORT  With the spike in Affordable Care Act health insurance costs, around 4·8 million people could lose coverage. Susan Jaffe reports from Washington, DC. 
The failure of Congress to continue premium subsidies is expected to make health insurance unaffordable for millions of Americans who do not qualify for other government insurance programs.
